Efficacy of Video Assisted Thoracoscopic Surgery (VATS) for a Secondary Spontaneous Pneumothorax

Abstract
Purpose: Purpose: Video assisted thoracoscopic surgery (VATS) has been widely used in thoracic surgery, especially for a primary spontaneous pneumothorax (PSP). Although VATS can be performed for a secondary spontaneous pneumothorax (SSP), there are few reported clinical studies. The purpose of this report is to describe the surgical outcomes and efficacy of VATS for an SSP.

Methods: A total of 135 cases of VATS were performed in pneumothorax patients between January 2004 and December 2006. Patients were divided into two groups based on undertaking surgery for a PSP (n=98) or SSP (n=27), and patient clinical characteristics were evaluated. VATS was performed using a vicryl endoloop and blebs were electrocauterized and followed by phrenic nerve crushing. Hospital stay and complications that developed in patients were compared between the PSP and SSP groups. Additional analysis was performed to evaluate VATS completion and thoracotomy conversion in the SSP group of patients.

Results: The SSP group of patients showed a longer duration of hospital stay and more frequent postoperative bleeding than the PSP group of patients(p=0.028). Conversion to thoracotomy was performed for 15 patients in the SSP group (conversion rate=40%). Patients in the SSP group with VATS completion demonstrated a short duration of hospital stay than patients with thoracotomy conversion. There was has no remarkable difference for complications and the recurrence rate between the groups.

Conclusion: Although VATS for an SSP did not shown an outcome as successful as VATS for a PSP, VATS is a safe and feasible procedure to perform for an SSP.
